States/Territories With Mandatory Multiple Worksite Reporting Employers with more than one worksite must file unemployment insurance contribution and wage reports with each state, and they must also file quarterly employment and wage reports with the state agencies that break down the information by industry and locality. All states are responsible for collecting employment data and providing it to the federal Bureau of Labor Statistics (BLS), but not all of them require employers to file a report containing the information. The BLS uses the information collected to analyze the U.S. employment situation. To facilitate the collection of this information, the BLS developed standardized Form BLS 3020, Multiple Worksite Report (MWR). The MWR is customized to some degree for each state and provides at the top of the form whether reporting is mandatory or voluntary in that state. Forms for each state are available on the BLS website. The BLS encourages employers to file multiple worksite information via electronic file transfer or web reporting. Information on the methods and specifications is available on the BLS website. To use the web reporting option, employers need to sign up for an MWR ID and password. The following chart lists the states and territories where MWR reporting is mandatory (this updates The Payroll Source®, §7.2-7), along with contact information for the state and territory (the name of each responsible state agency and phone numbers). If a state or territory is not listed here, the use of MWR is voluntary in that jurisdiction.
